Thank you for visiting the website of the Department of Otolaryngology and Head and Neck Surgery at Asahikawa Medical University.
First, let me introduce myself. I was born in Ashoro-cho, Ashoro-gun, Hokkaido, and after graduating from Hakodate La Salle High School, I graduated from Asahikawa Medical University in 1994 as part of the 16th class. After that, I joined our medical department and, under the warm guidance of many seniors, juniors, and staff, I have been working in my current position since January 2024. Although I am not very knowledgeable or talented, I will do my best with my inherent tenacity. Thank you for your cooperation.
Otolaryngology/Head and Neck Surgery literally covers the vast area of the ears, nose, oral cavity, pharynx, larynx, and head and neck. These areas are responsible for many functions necessary for maintaining life and maintaining its quality, such as hearing, balance, smell, taste, breathing, speech, and swallowing. Our role is to treat the wide variety of diseases that occur in this area without impairing these functions as much as possible. To do this requires a great deal of knowledge and skills, and we are studying hard day and night.
Due to the geographical characteristics of our hospital, which covers a vast area covering northern and eastern Hokkaido, it is essential to train specialists who can treat a wide variety of diseases so that treatment can be completed as much as possible for each region. Our department's training program has been created with this in mind, and each affiliated hospital has excellent specialists who are responsible for the medical care of the region. In addition, if highly specialized treatment is required, we will refer you to a university for treatment. In addition to standard surgeries such as cochlear implant surgery and head and neck free reconstruction surgery, our department also performs cutting-edge treatments such as endoscopic otological surgery, endoscopic thyroid surgery, and endoscopic salivary stone surgery. We are constantly updating our knowledge and technology and working hard to provide medical care that satisfies our patients.
Our department is not only focused on clinical practice, but also on research. Immunotherapy for head and neck cancer is currently being adapted to many antitumor drugs, and is continuing to make progress. We have been actively conducting immunological research into head and neck cancer for some time, and have gained a great deal of knowledge that can be applied to treatment in the future. In addition, basic and clinical research into tonsillar focal disease continues to be a theme of our department, and we are proud to be at the top level internationally.
Our department is made up of young staff, but I am proud to say that we have a team of highly motivated and excellent staff members. Together with them, we will operate a department that is highly active in clinical practice and research, and is filled with passion and sincerity. We will work together as a team to make our department a place that will lead the field of otolaryngology and head and neck surgery in the future, so that people will say that Asahikawa Medical University is in the north. Our history


旭川医科大学耳鼻咽喉科学教室は昭和51年10月に、海野徳二初代教授の着任により開設された。平成10年11月に本学第4期卒業生である原渕保明が第二代教授として就任した。開設当初は5名でスタートを切ったが、その後順調に医局員が増加し、現在では総勢70名を越える大所帯となっている。その間、道北、道東のみならず北海道全域にわたり、多くの人材を輩出し、各地域の耳鼻咽喉科診療の中核として機能する体制が整っている。2024年1月1日からは当教室生え抜きの髙原幹が第三代教授となり、臨床、研究、教育全てにおいて、さらなる高みを目指して教室を運営している。
教室は現在(2024年4月)、教授以下、准教授2名、講師2名、講師(学内)1名、助教3名、医員5名、海外留学2名、言語聴覚士2名、医局秘書2名の総勢20名で組織されている。
診療の面では聴力改善手術、鼻科手術、口腔咽頭手術、および頭頸部腫瘍手術を精力的に行い、北海道の耳鼻咽喉科診療の中心的役割を担っている。また、高度難聴に対する人工内耳手術、頭頸部癌に対して嚥下や発声機能を温存して治療するための微小血管吻合を用いた遊離再建手術や超選択的化学動注・同時放射線療法などの最先端の治療も行っている。上気道感染症や呼吸生理に関する疾患についても扁桃病巣疾患、睡眠時無呼吸症候群、アレルギー性鼻炎、小児中耳炎を中心に専門外来を開設し、病態解明の研究とその結果に基づいた最新の診療を行っている(診療概要の欄を参照)。
研究の面では「研究と臨床は互いにフィードバックするものでなければいけない」という教室の方針のもとで、1. IgA腎症や掌蹠膿疱症の病態解明、2. 頭頸部癌の新たな免疫治療の開発、3. 頭頸部癌の微小環境における病態解明、4. アレルギー性疾患のT細胞応答を中心とした病態解明、5. 好酸球性疾患の病態を上皮系サイトカインを中心に解明などのテーマで日夜研鑽している、などのテーマで日夜研鑽している(研究概要の欄を参照)。
関連病院としては、旭川市内のみならず、札幌市、江別市、函館市、苫小牧市、室蘭市、北見市、釧路市、富良野市、稚内市、名寄市、士別市、根室市、紋別市、遠軽町、芽室町、岩内町、雄武町と北海道全域にわたって基幹病院に常勤あるいは非常勤で教室員を派遣し、その地域の耳鼻咽喉科医療に大きく貢献している(関連病院の欄を参照)。
海外の研究留学施設としては、アメリカ合衆国のハーバード大学、メイヨー・クリニック(ロチェスター、スコッツデール)、ヴァンダービルト大学(ナッシュビル)、アーカンソー州立大学(リトルロック)、ジョージア・リージェンツ大学癌センター(オーガスタ)、南フロリダ大学モーフィット癌センター(タンパ)、スイスのチューリッヒ小児病院、スウェーデンのカロリンスカ研究所(ストックホルム)に多数の教室員を研究留学させている(研究概要の欄を参照)。
教室外活動としては、道内の耳鼻咽喉科医を対象として年に3回、北北海道耳鼻咽喉科懇話会を開催し、最新の知識の獲得と会員の親睦がはかられている。

What we do about basic study
当教室では耳鼻咽喉科・頭頸部外科領域を幅広く網羅する基礎研究を行っている。また全ての教室員は1つ以上の研究テーマを持ち、研究と臨床を互いにフィードバックさせ、双方のレベルの向上を図っている。毎年、英文学術誌に多くの論文が掲載され、それらの研究成果に対し、国内はもとより国外からも高い評価を受けている。当科で行っている臨床研究に関してはこちらを参照ください。
また、医局員に対して海外研究留学を積極的に推奨している。2000年度から、アメリカ合衆国のローズウェルパーク癌研究所(バッファロー)、ヴァンダービルト大学(ナッシュビル)、アーカンソー州立大学(リトルロック)、メイヨークリニック(ミネソタ、アリゾナ)、南フロリダ大学モフィット癌センター(タンパ)、ハーバード大学(マサチューセッツ)、スウェーデンのカロリンスカ研究所(ストックホルム)、スイスのチューリッヒ小児病院に延べ18名の教室員を研究留学させている。
I. 免疫アレルギー感染症分野
(高原 幹、熊井琢美、大原賢三、山木英聖、河野通久、脇坂理紗、佐藤遼介、井上貴博)

扁桃病巣疾患の病態解明
扁桃病巣疾患は「扁桃が原病巣となり、扁桃から離れた臓器に反応性の器質的または機能的障害を引き起こす疾患」と定義され、当講座では扁桃摘出術がその症状や所見の改善に有効であることを示してきた。
代表的疾患として掌蹠膿疱症、IgA腎症、胸肋鎖骨過形成症が知られており、その病態は不明であったが、当講座をはじめとした近年の基礎研究の進歩により、掌蹠膿疱症ではαレンサ球菌、IgA腎症ではパラインフルエンザ菌や細菌に普遍的に存在するCpG-ODNに対する過剰免疫応答が、扁桃T細胞上の皮膚、腎ホーミング受容体の過剰発現やT細胞非依存性B細胞活性化因子の発現亢進によるIgAの過剰産生を促し、皮膚や腎の病態形成や維持に関与していることが明らかになってきた。
さらに、種々の病巣疾患における扁桃摘出術の効果に関して臨床的に検討し、その結果は、扁桃病巣疾患診療の手引きにまとめられている。本研究分野は当科が長年継続して行なっており、我々が先頭に立って牽引してきたものと自負している。また、本邦独自と言える本疾患概念の研究を絶やさないためにも、我々が地道に継続していかなければならない責務を感じている。
II. 腫瘍免疫・血液病態学

頭頸部悪性腫瘍について分子腫瘍学的、腫瘍免疫学的およびウイルス学的な多面的解析を行い、将来のがんワクチン、分子標的治療、遺伝子診断・治療を含めたテーラーメイド診療の開発を目指している。
頭頸部癌における新たな免疫治療の開発
頭頸部癌の標準治療は手術、放射線療法、抗癌剤による集学的治療が主軸であるが、第4の治療法として免疫療法が頭頸部癌においても有用性が示された。免疫チェックポイント阻害薬が頭頸部癌にも適応となったが奏功率は2割前後と満足できるものではなく、より有効な免疫療法の確立が求められている。
免疫チェックポイント阻害薬はその免疫賦活化作用が腫瘍非特異的であることが問題点の一つである。よりT細胞を抗原特異的に増殖させるため、我々はペプチドワクチン療法に着目している。当講座ではこれまでEGFRやHER-3、c-Met、MDM2、p53、FGFR1など頭頸部癌に発現している腫瘍抗原を数多く同定し、その腫瘍抗原から実際に抗腫瘍T細胞を惹起可能なエピトープペプチドの同定を10種類以上行ってきた(右図参照)。

T細胞の増殖にはペプチドによるT細胞受容体刺激(シグナル1)に加えて、共刺激分子(シグナル2)とサイトカイン(シグナル3)による刺激が同時に入る必要がある(下図参照)。当講座ではペプチドワクチンの臨床応用を見据えて、有効な免疫アジュバントの探索とペプチドワクチン療法の開発を目指している。

III. 免疫・アレルギー病態学

シラカンバ花粉症、口腔アレルギー症候群の病態解明と
新規治療法の開発
当講座では欧米では最も多い花粉症であり、北海道における主要な花粉症であるシラカンバ花粉症に関しても研究を行ってきた。花粉症患者を含めたアレルギー患者数は増加の一途をたどっているが、いまだに根本的な治療法が見出されていないのが現状である。当講座ではアレルギー疾患の新たな治療法の開発とT細胞動態を中心に病態の解明をテーマとして研究を行っている。副鼻腔炎、アレルギー疾患の発症のメカニズムは明らかではなく、好酸球性疾患の病態解明に向けて上皮系サイトカインを中心に解析を行い、臨床的・基礎的検討を重ね解明を進めている。
これまでに当講座ではシラカンバ花粉症における抗原提示細胞についてcDNAアレイを用いて解析し、サイトカイン・ケモカイン産生ならびに共刺激分子発現を健常者と比較検討した。またシラカンバ花粉症に対するペプチドワクチン療法の開発に向けて、シラカンバの抗原エピトープの解析を行い、その一つが抑制性T細胞を誘導可能な事を見出し報告した。
現在では、近年同定されたTSLP等に代表される新規アレルギーサイトカインに関しての各ヘルパーT細胞サブセットや抗原提示細胞へ及ぼす影響に関する解析も進めている。当科は2008年より日本アレルギー学会認定施設となっており、シラカンバ花粉症患者に高率に合併する口腔アレルギー症候群(oral allergy syndrome:OAS)についての臨床的、基礎的検討も精力的に推し進めていく予定である。
当講座ではメイヨークリニックへ2名の研究留学経験があり、花粉食物アレルギー症候群(PFAS: pollen food allergy syndrome)の免疫学的機序の解明や気道炎症と神経系のクロストークについても継続して研究を行っている。PFASは花粉抗原と食物抗原の交差反応性によって生じ、原因花粉抗原は シラカバ主要抗原Bet v1が最も多い。現在のPFASの治療は原因食物の回避のみであるが、新規治療法の開発に向けて検討を行っている。

So far, 15 members of our department have studied abroad at eight research facilities in Europe and the United States and returned to Japan. One more member began studying abroad in April 2020. Members who returned from studying abroad have made the most of the cutting-edge knowledge and techniques they acquired, achieving many results. Our department will continue to actively conduct collaborative research with overseas facilities and promote cutting-edge research.
